Radiopharmaceuticals for Advanced Prostate Cancer

The power of advanced diagnostics. 

When treating cancer, seeing is everything. If there is risk your prostate cancer will or has spread to other parts of your body, Mayo Clinic experts use PSMA-PET scan to better find your cancer. This full-body molecular scan allows your care team to see precisely where the cancer is, if it has spread and if your treatment is working. The scan uses a special dye with radioactive tracers that illuminate hard-to-find cancer cells so your cancer can be found earlier, and your therapy can be adjusted based on each individual tumors' response to treatment. 

Life-prolonging therapies designed to minimize side effects. 

  • PSMA-targeted therapy is now available for even more people with PSMA+ metastatic castrate-resistant prostate cancer. A radiopharmaceutical, lutetium LU 177 vipivotide tetraxetan targets and sticks to cancer cells to precisely deliver treatment. Mayo Clinic experts have been involved with developing this therapy and have more experience with itthis therapy than any other center in the world. In March 2025, the FDA approved this therapy to be used after 

 one androgen receptor pathway inhibitor (ARPI) and now before chemotherapy, tripling the number of people eligible.

  • Hormone therapy can cut off the supply of testosterone to cancer cells causing them to die or grow more slowly.
  • PARP Inhibitors are a type of targeted therapy that attacks cancer cells, causing them to die. In metastatic prostate cancer, these medicines might be used when hormone therapy is no longer working.
  • Chemotherapy is sometimes used with hormone therapy or could also help treat advanced prostate cancer when hormone therapy isn’t working.
  • Stereotactic body radiation therapy uses high doses of radiation aimed at different angles to precisely target the tumor and limit radiation exposure to healthy tissue and can be used to address sites of cancer spread.
